Practice Standards 2.0 for Acute Hospital Care at Home

The Users Group Practice Standards Council developed the first national practice standards for Hospital at Home in 2023, as a supplement to research published in the Journal of the American Geriatrics Society. You can find the original research paper here.

In October 2025, the Council released an updated version, HaH Practice Standards 2.0, which are available for use with permission. Fill out this survey for access.

Ongoing Projects

Site Visit Exchange Program

The Hospital at Home Users Group Practice Standards Council has launched a HaH Site Visit Exchange Program across the United States and Canada.

The focus of this program is facilitating reciprocal, self-funded site visits (local or virtual), where programs can assess each other based on the HaH Practice Standards (Journal of the American Geriatrics Society, March 2025). This program is open to all active Hospital at Home Programs regardless of size or maturity.

Opportunities for participating programs:

For the Visiting Team: Gain an in-depth understanding of a peer HaH program’s successes and challenges. Visitors may be invited to share their expertise via a presentation, leading workshops, or conducting Grand Rounds.
For the Host Team: Receive a confidential, constructive assessment from a trusted peer HaH program and gain insights on your own program.
For Both Programs: Build new and lasting professional relationships, expand networks for problem solving for your HaH program, and work together on improving HaH patient care.

Our next cycle for the Site Visit Exchange Program will begin in January of 2027.
