Minimum Data Set
The Hospital at Home Users Group (HaHUG) and Quality Indicators Council launched the Minimum Data Set (MDS) initiative in 2022. The MDS is a de-identified, patient-level dataset that seeks to capture information on all patients enrolled in Hospital at Home (HaH) programs nationwide, dating back to November 2020.

Data Dictionary
Access Version 1 of the Data Dictionary here. Version 1 was published in March of 2022 by the Quality Indicators Council. Please note that this version is only to be used for data submissions of patients cared for between 2020 and 2024.
Access Version 2 of the Data Dictionary here. Version 2 was published in August of 2025 by the Quality Indicators Council. Please note that this version is only to be used for data submissions of patients cared for in 2025. Data submissions of 2025 data will be due March 31st, 2026.
