The Hospital at Home Users Group, in partnership with the American Academy of Home Care Medicine and the American Hospital Association, is pleased to present a webinar series for hospital and system leaders interested in learning about implementation opportunities and challenges associated with the new CMS waiver supporting Acute Hospital Care at Home.

In this session, Tom Kiesau of The Chartis Group will share perspectives on how a health system should approach developing the financial model around its hospital at home program. The session will:

  • Share insights on to how to define explicit clinical population scoping, both at program launch, and expanding/evolving over time.
  • Show how to estimate programmatic costs over a 3-5 year program planning horizon.
  • Demonstrate how to quantify near and long-term economic value creation enabled by the launch of a hospital at home program.
  • Explore common pitfalls and mistakes health systems make when planning for hospital at home.
