The Hospital at Home Users Group 2025 Annual Meeting, Leveling Up: Investing in Hospital at Home and the Future of Healthcare, is the nation’s most comprehensive digital HaH conference. Scheduled for October 16th, our programming includes an up-to-the-minute policy update, a provocative keynote, networking opportunities, presentations of cutting-edge research, awards showcasing the best of the HaH community, and more.  

Leveling Up features two concurrent tracks for developing and experienced programs. Each session will be packed with practical advice for your program, followed by Q&A.  

CME will be available.

We are proud to partner with the American Academy of Home Care Medicine (AAHCM) to make October Hospital at Home Month! If you want to continue your digital HaH conference experience in-person, register for the AAHCM Annual Meeting in New Orleans on October 17th and 18th.

The Hospital at Home Users Group Virtual Annual Meeting 2025 seeks to feature HaH leaders from around the country–and that means you! There are so many ways that you can be involved and showcase your exciting work on Hospital at Home. Submit a poster abstract, award nomination or photo/video today!

poster abstracts

To contribute to the Users Group Annual Meeting’s program, we invite you to submit a poster abstract in one (or more) of three categories: 

  • Research 
  • Clinical Vignette 
  • Single Practice Innovation 

Scientific abstracts should present the results of purposeful and systematic research aimed at better understanding the field of home-based medical care (e.g., epidemiological investigations). These abstracts must have replicable methods, clear analytic approaches, and data for presentation. 

Clinical abstracts should be either clinical vignettes (e.g., unusual clinical findings) or practice innovations (e.g., operational improvements). Practice innovations should provide replicable ideas for implementation at other home-based medical care practices.  

Those that submit the best abstracts in each category will be invited present their poster at our Annual Meeting and/or to make a poster available to view during the conference and on our website after the meeting.

Those who wish to submit an abstract are not required to attend the Annual Meeting. Please note that abstracts submitted to the larger AAHCM Annual Meeting or the World Hospital at Home Congress can also be submitted to the HaH Users Group Annual Meeting

The deadline for all submissions is 5pm EST on July 1st.

Awards

We are calling for nominations for Clinician of the Year and Program of the Year Awards!

The Hospital at Home Clinician of the Year Award recognizes a clinician who has made outstanding contributions to the delivery of quality Hospital at Home care. The Hospital at Home Clinician of the Year is someone who has gone above and beyond in delivering care and/or building and improving a hospital at home program. A clinician may be defined to include physicians, paramedics, nurse practitioners, physician’s assistants, nurses, social workers, skilled therapists. The recipient must deliver care in a Hospital at Home Users Group member program (see website for a list of member sites).

The Hospital at Home Program of the Year Award recognizes an exceptional hospital at home program that has demonstrated innovative approaches and sustained excellence in the delivery of hospital at home care. Again, the recipient program must be a member program of the Hospital at Home Users Group (see website for a list of member sites).

Nominations for the Clinician & Program of the Year Awards are due by 5 pm EST on July 1st.
