HaH User Group’s Virtual Annual Meeting Announced

Join us October 22

RECONNECT, RE-ENGAGE, RECHARGE

Join us for the HaH Users Group Virtual Annual Meeting, an opportunity for active HaH programs to interact with leaders in the field and attendees from other successful HaH programs.

Program highlights include educational offerings such as HaH in the COVID era, a State of the State of HaH address, a virtual poster session, and other interactive exercises designed to help you advance and build momentum around your HaH program.

The meeting will run via Zoom from 11 AM-5 PM (ET) on October 22, 2020. HaH Users Group members will receive registration information via e-mail in July, including instructions for submitting a poster.

As an additional benefit, the Users Group will cover the cost of registration for the October 23-24 AAHCM meeting for up to three members of each Users Group site. You will receive a special registration code when you sign up for the Users Group Annual Meeting.

Please note: There is no fee to attend the HaH Users Group Annual Meeting, but you must be an active member of the HaH Users Group.

A VIRTUAL OPPORTUNITY TO GET REAL ABOUT HOSPITAL AT HOME

For those who are not yet members of the Users Group, but who wish to learn more about Hospital at Home, including those who are contemplating starting up a HaH program, please join the afternoon session of the Virtual Annual Meeting from 3PM – 5PM (ET) on October 22, 2020.

In addition to hearing presentations from leaders in the field, there will be high-impact, interactive discussions with burgeoning and successful HaH programs.  This digital gathering is an intensive opportunity to engage HaH program leaders and get practical knowledge about what it takes to develop, run and grow a vibrant HaH program.

Mount Sinai Implementation Manual Now Available

Mount Sinai Hospitalization at Home and Rehabilitation at Home is offering its Implementation Manual through the HaH Users Group to hospitals and health systems interested in launching or building their HaH offerings.

Developed from Mount Sinai’s experience creating the Mobile Acute Care Team model with support from CMMI, the manual offers a brief history of the hospital at home model and describes Sinai’s experience creating a hospital at home platform that has been adapted to include rehab and observation unit patients. It also provides guidance on staffing, setting up the EMR, contracting, protocols, screening, referrals and intake. The nuts and bolts of Hospitalization at Home and Rehabilitation at Home visits are reviewed as well.

The manual should serve as an invaluable resource for hospitals looking to set up a new program and participate in the HaH CMS waiver program, as it covers the whole range of practical and operational considerations around setting up a high quality hospital at home program.
