
The Hospital at Home Users Group 2026 Annual Meeting returns virtually, once again convening HaH champions and clinical teams from across the country for the nation’s premier HaH event. The gathering will once again serve as the official preconference to the American Academy of Home Care Medicine (AAHCM) Annual Meeting, taking place November 4–6 in Indiana.
Join us on November 4 for Opportunity Knocks: Welcoming the Future of Hospital at Home, a dynamic day featuring new and engaging special sessions, peer consultations, and expert discussions on the latest innovations in acute care at home. This meeting will include:
- A keynote address from Joseph Smith, MD, PhD, a cardiologist and health innovation leader with deep expertise at the intersection of technology, policy, and home-based care
- An up-to-the-minute policy update
- Two dedicated tracks – one designed for programs just getting started with HaH and one for more experienced programs looking to scale their effort
- Research presentations and awards celebrating excellence in the HaH community
- Networking opportunities, and more

poster abstracts
To contribute to the Users Group Annual Meeting’s program, we invite you to submit a poster abstract in one (or more) of three categories:
- Patient Case Studies
- Practice Innovations
- Research Abstract
Patient Case Studies should present compelling patient case studies that highlight unusual, instructive, or noteworthy clinical findings encountered in the Hospital at Home setting. These submissions should offer clear takeaways that other HaH practitioners can apply to their own clinical practice.
Practice Innovations should showcase operational improvements or program innovations that have enhanced the delivery of Hospital at Home care. These submissions should present replicable ideas with at least preliminary outcomes that other HaH practices can adapt and implement in their own settings.
Research abstracts should present the results of purposeful and systematic investigations aimed at advancing the field of Hospital at Home care. These submissions must include replicable methods, clear analytic approaches, and actual data. Abstracts without results will not be considered.
Those that submit the best abstracts in each category will be required to present their poster at our Annual Meeting and make a poster available to view on our website after the meeting.
Those who wish to submit an abstract are not required to attend the Annual Meeting. Please note, if you have already submitted a poster to the AAHCM Annual Meeting, you can resubmit it here.
The deadline for submissions is July 13th, 5pm EST.
Awards
We are calling for nominations for Clinician of the Year and Program of the Year Awards!
The Hospital at Home Clinician of the Year Award recognizes a clinician who has made outstanding contributions to the delivery of quality Hospital at Home care. The Hospital at Home Clinician of the Year is someone who has gone above and beyond in delivering care and/or building and improving a hospital at home program. A clinician may be defined to include physicians, paramedics, nurse practitioners, physician’s assistants, nurses, social workers, skilled therapists. The recipient must deliver care in a Hospital at Home Users Group member program (see website for a list of member sites).
The Hospital at Home Program of the Year Award recognizes an exceptional hospital at home program that has demonstrated innovative approaches and sustained excellence in the delivery of hospital at home care. Again, the recipient program must be a member program of the Hospital at Home Users Group (see website for a list of member sites).
Nominations for the 2026 Clinician and Program of the Year Awards are due by July 13th, 5 pm EST.
Photo & video contest
We are always looking for new ways to show the impact that hospital at home has on patients, clinicians, and communities. Please submit a photo or video for the 2026 Photo and Video Contest that captures the power of this model by July 13th, 5 pm ET. We look forward to seeing your submissions.

- Login to your AAHCM memberclicks account (“Existing Users”) or create a “New User” account to register.
- At the top of the next page, click REGISTER YOURSELF and a form will appear below. Enter your FIRST NAME and LAST NAME, then indicate whether or not you are registering as part of a group. (If you are registering as part of a group, you will need to enter your group code upon checkout. If you do not have a group code, please contact info@aahcm.org BEFORE completing your registration.)
- Under Meeting Participation, ensure the “Hospital at Home Virtual Annual Meeting Registration – $200.00” option is selected.
- Populate the form with your information. Click to acknowledge the Registration and Cancellation Policy, then hit NEXT.
- On the following page, you will have the opportunity to input a discount code. Confirm your order and information. Click NEXT.
- Select how you would like to pay. If you choose the “Invoice me” option payment is required within 30 days; the “Pay now” option requires a credit card.
- You will then be directed to a confirmation page and receive a confirmation email with the amount paid and/or remaining balance.
Group Registration
- Create your group – this is defined as five or more attendees from the same institution.
- Contact info@aahcm.org. Our partners at AAHCM will give you a special code that you can distribute to members of your group to use when they register individually. Or, if you prefer to register all of the members of your group at once, our AAHCM colleagues will work with you directly to do that.

As in years past, the HaH Users Group Annual Meeting will be a virtual pre-conference for the in-person American Academy of Home Care Medicine Annual Meeting, which will be held November 5-6 in Indianapolis, Indiana.
Please note: If you or your group will be attending the AACHM meeting in-person in Indianapolis, you will have to register separately for both meetings.

The World Hospital at Home Congress (WHAHC) 2027
WHAHC 2027 is the leading gathering of international thought leaders, experts, and innovators in Hospital at Home. It will take place on February 25-27, 2027 in Sweden. By bringing together diverse perspectives and expertise, WHAHC aims to foster a global dialogue that will spearhead further advancements in the Hospital at Home model.
