Caregiver Innovation Projects
The Hospital at Home Users Group (HaHUG) is pleased to launch the first cohort of the HaHUG Caregiver Innovation Projects (CIP) for the development of Family Caregiver Support Tools. The HaHUG will provide up to three HaHUG member programs $5,000 each to develop, test, implement, and disseminate practical tools that support family caregivers in Hospital at Home programs.
Family caregivers are essential partners in care delivery, often helping to coordinate and support their loved one’s daily needs during the acute HaH episode. CIPs are designed with the intent to better support family caregivers through the development of practical, usable tools that respond to real-world caregiver needs in HaH.
CIPs should meaningfully engage caregivers with lived HaH experience as co-partners in the design, testing, or refinement process. Final tools and lessons learned will be disseminated throughout the broader HaHUG community to support adaptation and uptake across HaH programs.
