Caregiver Innovation Projects

The Hospital at Home Users Group (HaHUG) is pleased to announce the selected programs for the 2026 Caregiver Innovation Project cohort: Penn Medicine and Hackensack Meridian Health. The CIP is a HaHUG initiative designed to strengthen family caregiver supports across HaH programs. Selected member programs receive $5,000 each to develop, test, implement, and disseminate practical tools that respond to real-world caregiver needs, with final tools and lessons learned shared throughout the broader HaHUG community.

Both programs were selected for their commitment to developing practical, caregiver-centered tools that respond to real-world needs in Hospital at Home.

Penn Medicine will develop an on-demand caregiver skills curriculum adapted from the evidence-based Veterans Affairs Caregivers FIRST program, delivering short, tailored training modules accessible to caregivers during and after patient admission.

Hackensack Meridian Health will create a caregiver welcome guide and educational tools addressing cultural competency and health literacy, leveraging AI to develop animated videos that communicate with caregivers in the language and reading level most appropriate for them.

A new call for proposals for the 2027 CIP cohort will be launched in 2027. Stay tuned for further updates and announcements.
