This community of practice connects nurses across Hospital at Home programs and strengthens the nursing voice within this evolving model of care. The community provides a space for nurses to collaborate, share practices and resources, identify common challenges, and learn from one another across organizations and roles.

Through shared learning, professional development, and nurse-led collaboration, the Community of Practice supports nursing practice, leadership, innovation, and the continued development of Hospital at Home care. Nurses across roles, organizations, and experience levels are encouraged to participate and help shape the future of nursing within Hospital at Home.

Call for leadership applications

The Nursing Community of Practice (CoP) is looking for experienced, enthusiastic, and creative leaders to join the Leadership Team. The Nursing CoP Leadership Team will meet regularly to plan community meetings, set broader CoP goals, and discuss member engagement and communications.
