The Hospital at Home Users Group, in partnership with the American Academy of Home Care Medicine, is pleased to present our latest webinar for hospital and system leaders interested in learning about the opportunities and challenges associated with providing acute care in the home.

What do we really mean when we say “home” in the context of hospital at home? Many HaH programs are exploring care delivery for patients residing in a range of home-like community-based settings. This strategy serves to broaden patient access to high-quality acute care beyond the hospital and can address gaps in care and improve outcomes for underserved groups.

In this webinar, panelists from Boston Medical Center, Saint Luke’s, and the Boston VA describe how their HaH programs are delivering care to patients in alternate settings, including transitional housing/shelter environments, assisted living facilities, and group homes.
