Unique feature| Collaboration with Mount Sinai Hospital – The Bronx VA team screens all patients and the Mount Sinai team provides the direct acute level care at home. Post-discharge, all patients receive care transition support services from a Bronx VA HIH Geriatrician and social worker until they are re-established with their primary care team through a follow up visit.
