Program supports patients with congestive heart failure, chronic obstructive pulmonary disease, cellulitis and community-acquired pneumonia.

Unique feature| Due to a lack of a VA Hospital in Hawaii, our program mainly works with Tripler Army Medical Center, which services our veterans at discharge.
