Building from the experience of two leading programs, this webinar explored the full range of patient policies and procedures that serve as core elements of a successful hospital at home program. Specifically, the webinar addressed how to find and attract patients with the acute conditions and living situations that make sense given your program’s capacity and capabilities. Our presenters also discussed the challenges and opportunities associated with identifying Hospital at Home-eligible patients in various settings—from the ER to the clinic and community.
